Medicare covers certain medically necessary equipment and supplies that your physician prescribes for your use in your home.
Professional Medical is able to assist you with obtaining certain needed medical supplies including:
- Urological Supplies such as catheters, drain bags, leg bags
- Wound Care Supplies such as bandages, dressings, gauze, tape
In order to bill your insurance we will need the following:
- Full Name
- Date of Birth
- Medicare Number
- Secondary Insurance information- if any
- Physician name and number
- Assignment of Benefit completed and signed
- Detailed information of the items required
